Finasteride Effective in Preventing Prostate Cancer

Cancer Prevention Trial (PCPT), finasteride (Proscar), an agent commonly used to treat lower urinary tract problems and baldness in men, was safe and effective in reducing the risk of prostate cancer. The findings were recently published published in the New England Journal of Medicine.1

“Finasteride is safe, inexpensive, and effective as a preventive strategy for prostate cancer,” principal investigator Ian Thompson, Jr, MD, chair of SWOG’s Genitourinary Cancer committee, and president of CHRISTUS Santa Rosa Hospital Medical Center, said in a press release. “Doctors should share these results with men who get regular prostate-specific antigen (PSA) tests that screen for the presence of prostate cancer. The drug will have its greatest effect in this group of men.”

The initial SWOG Cancer Research Network PCPT trial, which was funded by the National Cancer Institute, first opened for enrollment in January 1993. In the trial, 18,882 men aged ≥55 years with a normal digital rectal exam and a PSA level ≤3.0 ng/ml were randomized to receive 5 mg of finasteride daily (n = 4368) or placebo (n = 4692) for 7 years.

Patients were excluded from the first analysis if they had a prior diagnosis of prostate cancer (n = 2), had failed to complete the study due to early termination (n = 2585), died (n = 1123), refused end-of-study biopsy (n = 3927), were lost to follow-up (n = 1256), had data that was still in review (n = 64), or had an end-of-study biopsy after randomization cut-off (n = 865).

Of the 9060 men who were included in the first analysis, prostate cancer was detected in 18.4% of men (n = 803) in the finasteride arm and 24.4% of men (n = 1147) in the placebo arm, resulting in a 24.8% reduction in the prevalence of prostate cancer over the 7-year span (HR, 0.75; 95% CI, 18.6%-30.6%; P<.001).

The percentage of men who experienced sexual adverse events was higher in the finasteride arm, whereas men in the placebo arm were more likely to experience urinary symptoms. Notably, higher-grade tumors with a Gleason score ≥7 were more commonly observed in men in the finasteride group (37%) versus those in the placebo group (22.2%).2 These findings raised concerns over an increased risk of developing high-grade disease, which prompted the FDA in 2011 to issue guidance against its use for the prevention of prostate cancer.

However, subsequent analysis from the PCPT trial spanning an additional year in length revealed no significant difference in the rates of overall survival after a diagnosis of prostate cancer. Among the entire study population, prostate cancer was identified in 10.5% of men (989/9423) in the finasteride arm and 14.9% of men (1412/9457) in the placebo arm (HR, 0.70; 95% CI, 0.65-0.76; <.001). Of those diagnoses, 3.5% (n = 333) and 3.0% (n = 286) of patients were classified as having high-grade disease in the finasteride and placebo groups, respectively (relative risk, 1.17; 95% CI, 1.00-1.37; = .05).3

In this analysis, 5034 deaths occurred—2538 of which were reported in the finasteride group, while 2496 occurred in the placebo group. The 10-year survival rates were 83.0% in the finasteride arm versus 80.9% in the placebo arm for men with low-grade disease and 73.0% versus 73.6%, respectively, for men with high-grade disease. The 15-year survival rates were 78.0% and 78.2% with finasteride and placebo, respectively.

Researchers then paired study participants to the National Death Index, a database of death records overseen by the US Centers for Disease Control Prevention. At a median follow-up of 18.4 years, there were 3048 deaths in the finasteride arm and 2979 deaths in the placebo arm. Forty-two deaths in the finasteride arm and 56 deaths in the placebo arm were attributed to prostate cancer; of those deaths, 18 in the finasteride arm and 20 in the placebo arm were classified by a Gleason score ≥7, revealing no evidence of increased risk of high-grade prostate cancer with finasteride.

“There are significant negative consequences to patients’ health and quality of life that can result from prostate cancer treatment, as well as to their finances and their peace of mind,” said Thompson. “If we can save people from surgeries, and scores of examinations and tests, and spare them from living for years with fear, we should. The best-case scenario for patients is prevention, and this trial has found an inexpensive medication that gets us there.”

The researchers found that the mice treated with letrozole gained more weight and had increased insulin resistance compared with mice given placebo, but that cohousing mitigated these developments. Testosterone and luteinizing hormone levels rose for letrozole-treated mice housed together, whereas the cohoused mice had normal testosterone and LH levels and experienced normal estrous cycling and ovulation.

Thackray and colleagues also observed a shift toward similar gut microbiomes in the cohoused mice, which she said indicated that “there is an exchange of microbes between these two groups.” Additional analysis revealed that the bacteria Coprobacillus changed when comparing letrozole-treated mice housed together and letrozole-treated mice in the cohousing group, indicating that this specific bacteria “potentially could be a probiotic that we can test as a potential treatment for protecting or preventing the development of PCOS,” Thackray said during a press conference.

“This cohousing approach suggests that the changes that we see in the microbiome are not just a result of PCOS, but it may actually play a causal role,” Thackray said. “Fundamentally, this is suggesting that modulation of the gut microbiome could be a potential therapy for PCOS. Upfront Combo Shows Improved OS in Metastatic Breast Cancer

Women with metastatic hormone-sensitive breast cancer lived nearly 8 months longer with the addition of fulvestrant (Faslodex) to first-line anastrozole (Arimidex), long-term results of a phase III trial showed.

In the study of nearly 700 patients, overall survival (OS) in the combination arm was 49.8 months versus 42.0 months with anastrozole alone at 7 years median follow-up (HR 0.82, 95% CI 0.69-0.98, P=0.03), Rita S. Mehta, MD, of the University of California, Irvine, and colleagues reported.

At 5 years, 42% of patients in the combination arm were still alive compared with 33% in the anastrozole arm, despite nearly half the women in the control arm crossing over to fulvestrant, a selective estrogen-receptor down-regulator, after disease progression, they wrote in the New England Journal of Medicine.

Mehta told MedPage Today that until now, first-line metastatic breast cancer trials have shown improvements in progression-free survival (PFS) but not OS.

“If we don’t see a survival benefit with newer agents compared to old agents, then we haven’t really moved the needle,” she said. “Ultimately patients want to see if they can live longer or not.”

Early findings from SWOG S0226 were first reported by Mehta in 2011, at a median 3 years follow-up, and showed a significant PFS benefit with the combination (15.0 vs 13.5 months) and a trend toward improved OS.

“Some physicians changed their practice back then — some were not convinced,” Mehta said. “But now they should be convinced, because now we have one of the longest follow-ups and one of the largest studies.”

The current study, SWOG Cancer Research Network S0226, examined outcomes of 694 women randomized 1:1 to standard-dose anastrozole or the combination of anastrozole plus intramuscular fulvestrant. Patients were stratified by prior use of tamoxifen. All patients had hormone receptor-positive disease.

Further subgroup analyses for OS in the updated findings showed no benefit for the combination among those whose disease progressed from 5 to 10 years from their initial breast cancer diagnosis (HR 1.01, 95% CI 0.69-1.48) and in patients not sensitive to endocrine therapy (HR 1.08, 95% CI 0.64-1.80).

Mehta noted that with the longer follow-up, there were no additional grade 4/5 adverse events (AEs) on the combination arm. “In fact, if there was any additional grade 4 toxicity it was on the anastrozole arm,” she said.

Previous grade 4 AEs in the combination arm included arthralgia, dyspnea, thrombosis or embolism, and thrombocytopenia, each occurring in one patient, and there were three treatment-related deaths in the combination arm — all previously reported.

Grade 3 toxicities were similar between the two arms, at 15% of patients on the combination and 13% of those on anastrozole-alone (P=0.47). The most common grade 3 AEs (ranging from 1% to 4%) were fatigue, GI symptoms, hot flashes, mood alterations, and musculoskeletal pain. Treatment discontinuation due to AEs were rare — 12 patients on the combination and five on anastrozole alone.

Breast Cancer Drug Shown to Impair Brain Function

A study in nonhuman primates by researchers at the University of Massachusetts and the University of Maryland School of Medicine has shown that the breast cancer drug letrozole has adverse effects on the brain. Letrozole is an aromatase inhibitor (AI) that is often given as adjuvant therapy to women with estrogen receptor-dependent breast cancer, but the treatment is linked with side effects including hot flashes and mood changes. The new studies, carried out in both male and female marmosets, showed that letrozole therapy caused behavior changes consistent with central nervous system (CNS) symptoms, and negatively impacted on the physiology of neurons in the brain’s hippocampus region.

Reporting their studies in the Journal of Neuroscience, Nicole Gervais, Ph.D., and colleagues say their findings highlight the need both for greater understanding of the mechanisms that underlie how AIs impact on brain function, and also “the development of new treatment approaches for breast cancer patients that minimize adverse effects on the brain.” Their paper is titled, “Adverse effects of aromatase inhibition on the brain and behavior in a non-human primate.”

Estrogens are produced through the conversion of testosterone by the enzyme aromatase, and patients with estrogen receptor-dependent breast cancer are commonly treated using adjuvant aromatase inhibitors, such as letrozole, to prevent this conversion. AI therapy is also used for men with prostate or breast cancers. Unfortunately, AI drugs can cause side effects including insomnia, depression, hot flashes, and memory problems that may be severe enough for patients to discontinue treatment, the authors wrote. “However, the mechanisms by which AIs give rise to these CNS symptoms remain unclear,” they pointed out, as studies in humans often lack proper controls and are hampered by confounding factors such as concurrent chemotherapy/radiotherapy treatment, stress, and disease stage. There have also not been any systematic analyses of sex-related differences in side effects.

The researchers’ reported study was designed to investigate whether continuous letrozole therapy in the marmoset as a nonhuman primate model would cause similar side effects to those observed in humans, but without the muddying effects of confounding factors. Marmosets and humans have similar brain structural and functional organization, the researchers pointed out, and prior studies indicated that the two species also share similarities in sleep and thermoregulation patterns, cognitive ability, and anxiety profiles.

Nine male and seven female marmosets were given letrozole in their puddings for four weeks, and the effects of treatment on cognition, anxiety, thermoregulation, brain estrogen content, and hippocampal pyramidal cell32 physiology were assessed. “This was the first study using a nonhuman primate to examine the effects of letrozole on brain and behavior,” the researchers wrote.

The results in a nonhuman primate corroborate many of the symptoms reported by women taking AIs and reveal detrimental effects of these treatments on the brain, in part through elevated hippocampal E2 levels,” the researchers concluded. “Future studies are needed to elucidate the precise mechanisms by which AIs compromise the CNS.”

Researchers manipulated the levels of leucine in cultured cells and found that a tenfold increase in the amino acid enhanced the ER-positive breast cancer cells’ division.

Newswise — BUFFALO, N.Y. — One of the mechanisms that makes so-called triple-negative breast cancer (TNBC) notoriously aggressive and hard to treat is its ability to disarm proteins that suppress tumors and mutate them into tumor drivers. Seeking to better understand this process, a team from Roswell Park Comprehensive Cancer Center decided to focus not on what these TNBC tumors lack — the estrogen receptor (ER-alpha), progesterone receptor (PR), and HER2 proteins — but something they possess: an alternate estrogen receptor, ER-beta. What they found, in collaboration with an international team of researchers, and report in a new article in JNCI: Journal of the National Cancer Institute, is that a widely used breast cancer drug, tamoxifen, can turn the ER-beta protein into a double agent capable of disrupting the processes that typically make TNBC tumors so devastating.

Tamoxifen is approved for treatment and prevention of ER-alpha-positive breast cancers, but was previously thought not to be effective against TNBC tumors because it targets ER-alpha, one of the proteins they lack. But this new work from a team led by Gokul Das, PhD, Associate Professor of Oncology in the Department of Pharmacology and Therapeutics and Co-Director of the Breast Disease Site Research Group at Roswell Park, proposes a new role for this broadly studied and well-tolerated targeted agent: targeting ER-beta.

Mutation of an important tumor suppressor, the TP53 protein (also known as p53), is known to be the major mechanism behind most triple-negative breast tumors. In TNBC with normal TP53, ER-beta binds to normal TP53, disabling its tumor-suppressing capabilities and resulting in aggressive tumor behavior. But Dr. Das and colleagues show here that in TNBC with mutated TP53, ER-beta functions as a tumor suppressor by binding to mutated TP53, preventing it from functioning as a tumor driver.

“We demonstrate that ER-beta directly binds to TP53 and influences its function, and that what happens from there hinges on what type of TP53 the tumor cells have. ER-beta can have diametrically opposite functional capabilities depending on what type of TP53 — normal or mutated — is present in the tumors,” notes co-first author Chetan Oturkar, PhD, a research affiliate in Dr. Das’ lab.

The researchers report based on a series of preclinical studies that tamoxifen increases binding of ER-beta to mutated TP53 in TNBC leading to death of cancer cells, and their analysis of a large breast cancer patient database showed that high levels of ER-beta in TNBC with mutated TP53 are associated with a more favorable prognosis.

“We have uncovered what we believe is the first known prognostic marker for triple-negative breast cancer, opening up an important opportunity to repurpose tamoxifen to treat TNBC,” says Dr. Das. “We are particularly pleased and excited to see that a readily available drug that has safely been given to hundreds of thousands of patients with or at risk for other types of breast cancer may be used to treat a disease subtype for which there are no effective therapies at present.”

A new drug combo has been made available to some patients with advanced breast cancer on the NHS in England.

The National Institute for Health and Care Excellence has recommended the drugs – called abemaciclib (Verzenio) and fulvestrant (Faslodex) – in new draft guidance, after the treatments were shown to slow disease progression and delay the need for chemotherapy in a clinical trial.

NICE said the combo could be an option for up to 4,800 women. This would include patients whose breast cancer has spread and who have already had hormone therapy. Patients’ cancer cells would need to test positive for certain hormone receptors, but not the HER2 receptor.

The  combo will only be used where the alternative would be a combination of two other drugs, a hormone therapy called exemestane (Aromasin) and the targeted  everolimus (Afinitor).

Rose Gray, policy manager at Cancer Research UK, called the decision “great news.”

“Clinical trials showed promising results, with this treatment giving patients more time before their disease gets worse which is important,” she said.

Long-term data needed

Abemaciclib is a targeted cancer drug that works by blocking growth molecules inside cancer cells, which stops them from dividing. It’s been tested in combination with the hormone treatment fulvestrant.

In a trial involving 669 patients with advanced breast cancer, taking a combination of abemaciclib and fulvestrant increased the time before patients’ cancers got worse, compared to taking fulvestrant alone.

Patients taking the drug combo were alive for an average of 16.5 months without their cancer getting worse, compared to 9.3 months for those just taking fulvestrant.

But serious side effects were more common in the group taking the drug combination, with 1 in 5 people experiencing side effects including severe diarrhoea and blood clots.

The trial has not been running for long enough to know how much the drug combination will improve long-term survival for these patients.

“Because this treatment is so new, we don’t yet know how well patients do in the long term – but this drug has been approved through the Cancer Drugs Fund, which means patients can access the drug quickly while more data is collected about its longer-term benefits,” said Gray

The Cancer Drugs Fund gives patients access to new treatments before they are approved for routine use in the NHS. This gives the NHS and the drug company time to collect more data on the long-term benefits of the drug.

The drug combination will stay on the Cancer Drugs Fund while the final results of the clinical trial are collected, which is expected to be in February 2020, at which point NICE will decide whether the NHS should continue to fund the treatment.

Improving quality of life

Meindert Boysen, director of the centre for health technology assessment at NICE, said the committee heard from patients that treatments that postpone or avoid the need for chemotherapy are important and could improve quality of life.

“They also highlighted the importance of people remaining in better health longer, without the disease progressing,” he said.

NEW ORLEANS — Long-term testosterone therapy may not only help men with obesity and hypogonadism to lose weight, but it may have other useful effects on metabolic function and protect against obesity comorbidities, according to a presentation at the Androgen Society annual meeting.

“Going forward, we will see many more clinics that will be adopting to add testosterone therapy in addition to lifestyle modification for men who are obese and hypogonadal,” Abdulmaged M. Traish, PhD, professor of biochemistry and urology at Boston University School of Medicine, told Endocrine Today. “It’s not a light switch. It takes time. It takes several years to begin to see a considerable and consistent weight loss. If you ever want to remodel your kitchen, you cannot do it overnight. The same thing in biology. You don’t lose weight because all these fat cells are going to die overnight, and all these muscle cells are going to grow up overnight. It takes a gradual process.”

Traish presented the findings of a prospective registry study of 805 men with hypogonadism (mean age, 59 years) and their responses to testosterone therapy in relation to obesity and other metabolic measures. Weight status was varied in the total cohort, with 462 men with obesity, 280 with overweight and 63 with normal weight. A total of 412 men, including 273 with obesity, were treated with testosterone undecanoate injections at 12-week intervals for 10 years. The other 393 participants elected to go untreated and served as a control comparison for the treatment group.

After treatment, the researchers found that there was a noticeable decrease in the percentage of weight loss from baseline, especially after 2 years, in participants with overweight (P < .0001 after 2 years) and obesity (P < .01 through 2 years; P < .0001 for years 2 to 10) compared with those who went untreated. Testosterone therapy was associated with reduced mean body weight from 97.3 kg to 84.6 kg in the cohort with decreases in waist circumference (107 cm to 92 cm) and BMI (31 kg/m2 to 27 kg/m2). In addition, testosterone therapy led to improved HbA1c, blood glucose, total cholesterol and visceral fat levels, according to Traish, who said because the study included a control group and accurate adherence since the testosterone injections were made by a physician every 3 months, “this data is really significant and important.”

The researchers also monitored adverse events in the cohort and found that there were 12 deaths, but no myocardial infarctions or strokes in the men with obesity treated with testosterone compared with 57 deaths, 47 MIs and 44 strokes in men who were untreated. The rates of prostate cancer were 2.9% (n = 8) in the men with obesity who were treated with testosterone and 11.1% (n = 21) in the men with obesity who were untreated (P < .0005), although Traish said he found slight but expected increases in hemoglobin and hematocrit in those treated with testosterone.
